Workers' Compensation
Started in 2000, the LGC’s Property-Liability Trust Workers’ Compensation Program (WCP) focuses on managing claims fairly and helping employees get the care they need without hassles or delays. The WCP is not a managed care organization that directs injured workers to a specific physician, facility or pharmacy. Instead, employees are free to treat with providers of their choosing.
Integrated Benefits Management
Our integrated benefits management program features a proactive recovery plan centered on preserving an employee's work relationships and facilitating an appropriate return-to-work plan. An assigned nurse case manager serves as a valuable coordination resource by facilitating communication between the employer, the injured employee, the healthcare provider and any other involved parties.
Filing a Claim
Here are two ways you can file a workers’ compensation claim with the LGC:
- Use the NH First Report of Injury form sent to you from the LGC via secure e-mail. Complete the form and then save it as an Excel file. Reply directly to the secure e-mail, attach the completed form and send.
- Click on NH First Report of Injury form linked below as a fillable PDF. Complete the form, then print and fax it to the LGC’s Claims Department at 800.262.9507. (Please do not send scans of this form nor return it via e-mail. Thank you.)

Provider and Pharmacy Lookup Tool
