Do I need to notify LGC HealthTrust if I change my name, phone number, address or the number of dependents covered under my insurance plan with you?
Yes. It's important to notify our Enrollee Services Department as soon as possible if you need to make any of those changes. This will assure that our member records are accurate and you continue to receive timely LGC HealthTrust mailings. You can provide those updates to your employer group's Benefits Administrator for them to share with us using an LGC HealthTrust change form. You can also contact us directly by doing one of the following:

What is LGC HealthTrust?
The LGC HealthTrust is a nonprofit employee benefits pool devoted to providing a full range of employee benefit programs exclusively to schools, municipalities, counties and other quasi-municipal entities in New Hampshire. It provides employee benefits to more than 105,000 New Hampshire employees and their dependents, including medical plan options with comprehensive, prescription drug and dental care coverage plus health management programs, group life insurance, short- and long-term disability programs and workers' compensation. Founded in 1984 by the New Hampshire Municipal Association, LGC HealthTrust is a program of New Hampshire Local Government Center which is governed by a 31-member board of directors with school, county and employee representation. The LGC HealthTrust's benefit programs are administered or insured through highly respected companies, including Anthem Blue Cross Blue Shield, Northeast Delta Dental, EyeMed Vision Care, Health Resources, Caremark and Onlife Health (formerly known as Gordian Health Solutions).

Our 27-year partnership with Blue Cross and Blue Shield specifically offers LGC HealthTrust enrollees with comprehensive provider networks and worldwide recognition of their plan ID card as well as claims processing and customer service support.
Are there special discounts I can access from Anthem Blue Cross and Blue Shield?
Yes. As an Anthem member, you qualify for valuable discounts on an array of health and wellness-related products and services. To learn more, visit Anthem's website and use the Member Log In to sign in. Then click into the "Health & Wellness" section
and on the "Access your Discounts" link where you'll find savings on child safety products, fitness equipment, allergy-related items and more.
What should I do if I want to change my primary care provider (PCP) or if the medical plan ID card I receive has the wrong PCP listed for me or for one of my dependents?
To make PCP changes, or correct a PCP listing on your medical plan ID card, please call Anthem Blue Cross and Blue Shield directly at the toll-free service phone number listed on back of your ID card.
What benefits are employees who have been called to active or training duty as reservists entitled to upon returning to work after uniformed service?
Upon returning to work after uniformed service, individuals are generally entitled to all benefits they had at the time service began per the Uniformed Service Employment and Reemployment Rights Act of 1994 (USERRA).
