Health Awareness Program
New Hampshire Local Government Center (LGC) HealthTrust offers the Health Awareness Program to support your efforts to improve or maintain your health and reward you for achieving a healthier lifestyle. At LGC HealthTrust, we recognize that physical activity is part of a healthy life; but we also want you to understand that eating a balanced diet, maintaining a healthy weight and managing your stress are equally important. In 2012 and beyond, we challenge you not only to be physically active, but to work toward eating well, getting to and maintaining a healthy weight, and managing your stress. The Health Awareness Program is designed to help you achieve these goals.
Health Awareness Program Eligibility
Health Awareness Program reimbursements of up to $300 per calendar year are available to enrollees, spouses and retirees who complete the onmyway™ Health Assessment (HA) and for dependent children (if the enrollee completes the HA).
LGC HealthTrust medically-covered enrollees, spouses and retirees qualify for Health Awareness Program annual reimbursements of up to $150 for physical activity (including self-reported activity) and up to $150 for non-physical activity classes and counseling after completion of the HA.
Dependent Children Eligibility
Medically-covered dependent children are eligible for Health Awareness Program reimbursement once the medically-covered enrollee completes the HA.
Covered dependents under age 18 qualify for up to $300 in Health Awareness Program reimbursement; requests may include any combination of physical and non-physical activity. Activities are eligible for reimbursement if they are a group class, clinic or instructional camp with start and end dates and provide instruction. Examples of eligible classes include karate, swimming, CPR/First Aid and diabetes education. Examples of classes/programs that are not eligible include AAU or sports teams/leagues, one-on-one nutritional counseling, one-on-one personal training, self-reported physical activity, tobacco cessation and advanced programs.
Covered dependents age 18 and older qualify for Health Awareness Program annual reimbursement of up to $150 for physical activity and up to $150 for non-physical activity classes and counseling.

How to Get Reimbursed
You must complete the 2012 HA to be eligible for Health Awareness Program in calendar year 2012. When you complete the HA determines your date of eligibility for Health Awareness Program, as shown in the following chart:

Physical Activity Reimbursements—Self-Reported Activity, Classes and Facility Use
Receive up to $150 annually for a combination of self-reported physical activity (SRPA) and physical activity classes. This reimbursement opportunity can be used to help you to stay physically active and achieve a more consistent exercise routine. Use your reimbursement to help cover the cost of your gym membership or sign up for that aerobics class.
For the 2012 calendar year, reimbursement for fitness facility memberships is discontinued; however, individuals may be reimbursed for physical activity at a fitness facility by completing and submitting a SRPA Request Form. (Individuals may receive up to $37.50 for each quarter SRPA requirements are met.)
For Self-Reported Physical Activity (reimbursable for LGC HealthTrust enrollees, retirees, spouses and dependents 18 years and older):
- Participate in various activities that get you up and moving like hiking, taking a bike ride or heading to the gym. Track your physical activity to be rewarded for your efforts!
- Complete the Self-Reported Physical Activity Request Form verifying 30 minutes of physical activity took place on a minimum of 12 calendar days per month.
- Follow the form’s instructions for submission to receive up to $37.50 for each quarter you meet SRPA requirements.
- You must submit your completed Self-Reported Physical Activity Request Form quarterly
(four times per year; the submission deadline is two calendar months from the end of March, June, September and December).
For Classes
- Before you enroll, verify the training or class is eligible for Health Awareness Program reimbursement by reviewing our Approved Programs list or contacting LGC's Enrollee Services Department at 800.527.5001.
- Once verified, sign up for an eligible program and pay the course fee.
- Obtain a receipt of payment and a program description (brochure or printed web page) for submission with your reimbursement request.
- Attend at least 75 percent of the program’s sessions or classes.
- Have the program’s certified instructor sign a Class/Training Reimbursement Request Form and follow its instructions for submission.
- Class reimbursement submission deadline is two calendar months from the end of the program.
Examples of eligible physical activities include aerobics, boot camp, kickboxing, Pilates, self-reported physical activity, swimming and Zumba®. Examples of non-eligible activities include bowling league, rock climbing, sports teams/leagues, camps and mini-golf.
Non-Physical Activity Reimbursements—Classes and Counseling
Receive up to $150 annually for non-physical activity classes. This reimbursement opportunity can be used to address nutrition, stress management, tobacco cessation and other health/safety related topics. Combination programs that incorporate physical activity with nutrition or stress management, including combination nutrition/physical activity classes and yoga, also fall into this category.
For Classes
- Before you enroll, verify the training/class is eligible for Health Awareness Program reimbursement by reviewing our Approved Programs list or contacting LGC's Enrollee Services Department at 800.527.5001.
- Once verified, sign up for an eligible class or program and pay the course fee.
- Obtain a receipt of payment and a program description (brochure or printed web page) for submission with your reimbursement request.

Examples of eligible activities include CPR/First Aid, defensive driving, healthy cooking, anxiety and stress management, one-on-one nutritional counseling, parenting, child birth, Weight Watchers®; programs that combine physical activity with nutrition or stress management, such as Create Your Weight, Curves® Weight Management, one-on-one personal training and yoga.
